What Is a Geriatric Care Manager?

A Geriatric Care Manager (GCM) is a professional who specializes in helping older adults and their families manage the complex and often challenging issues associated with aging. GCMs provide a wide range of services, including comprehensive assessments, care planning, and coordination of services to address the unique needs of each client.

GCMs have extensive knowledge of the healthcare system, community resources, and long-term care options available to older adults. They work closely with their clients and their families to develop personalized care plans that address physical, cognitive, social, and emotional needs.

GCMs also act as advocates for their clients, working to ensure that their rights and interests are protected. They may help resolve conflicts with family members, healthcare providers, or other service providers. Additionally, they can provide support and education to family members and caregivers, helping them to better understand the needs of their loved ones.

Overall, geriatric care managers play an essential role in helping older adults maintain their independence, manage chronic conditions, and navigate the challenges of aging. They provide invaluable support to families who may be struggling to balance the demands of caregiving with their own needs and responsibilities.
